Millington Online Student Registration for Shared Residency Students

Current and New Shared Residency Students

 

A Shared Residence Affidavit is to be completed if residency requirements cannot be provided due to Parent(s) and child(ren) sharing a residence with the homeowner/lease holder seven days a week, year round. The affidavit must be re-certified through Student Services at MMSD Central Office each year. The homeowner must accompany the parent at Central Office with the appropriate proofs in order to comlete the affidavit. 

Once the Shared Residency Affidavit has been completed and approved at the MMSD Central Office, you may then complete the Online Registration process through the Skyward Parent Portal.

Homeowner/Lease holder must have a valid Tennessee Driver’s License AND two of the following proofs below:

  • Mortgage statement or deed to the home in which the student will be living
  • Lease agreement to the home in which the student will be living
  • Real Estate Tax Receipt
  • Public Assitance/Government Benefits Letter
  • Recent MLGW or City of Millington Water Bill

Parent/ Guardian must have a valid Tennessee Driver’s License AND two of the following proofs below bearing the same address as the homeowner:

  • Car Registration
  • Voter Registration Card (Online application is not accepted)
  • Payroll Stub
  • Bank Statement
  • Government Assistance Letter
  • Social Security Information
